Use this form to request access, correction, deletion, or anonymization review for information submitted through the Amalgus Group website.

The fastest way to request deletion or review of submitted information is to provide the Submission ID shown after your original submission. If you do not have the Submission ID, you may still submit a privacy request with the email used in the original submission, approximate submission date, submission type, and enough non-sensitive details for Amalgus Group to locate the record. Amalgus Group may request additional verification before deleting or anonymizing information.

Do not include credentials, government identification, medical information, financial information, confidential employer information, or sensitive personal information in this request.
